Arthroscopic PCL Reconstruction

Arthroscopic Posterior Cruciate Ligament Reconstruction

It is a minimally invasive surgery where your Torned Posterior cruciate ligament in the knee is replaced by your own tissue graft through tiny holes made on the knee using tiny instruments

What is Posterior cruciate ligament?

It is one of several ligaments that connect the femur (thighbone) to the tibia (shinbone). The posterior cruciate ligament keeps the tibia (Leg bone)from moving backward with relation to the thigh bone.

PCL Injury Mechanism

What causes a PCL injury?

A posterior cruciate ligament injury happens when a powerful force goes through the top of the tibia, pushing it backwards and thus stretching the posterior cruciate ligament.

PCL injuries are usually caused by:

  • 1. Car Accident:

Dashboard injuries, where the knee is bent and the shin hits the car dashboard pushing the knee into hyperflexion is the most common cause of PCL tears.

  • 2. Fall

Falling onto a bent knee with the foot pointing downwards also common sporting PCL injury.

  • 3. Sporting Tackle:

Sometimes the posterior cruciate ligament can be injured during a football tackle when the knee is bent.

  • 4. Hyperextension:

The posterior cruciate ligament can also be damaged by the knee bending too far backwards.

How do you Know you have got PCL(Posterior cruciate Ligament) injury

  • An injury to the knee by powerful force.
  • Developed Pain with swelling after the injury
  • knee stiffness and may cause a limp
  • Difficulty walking
  • The knee feels unstable, like it may "give out"

Surgeon Consultation

Our surgeons will examine you thoroughly and diagnose the posterior cruciate ligament tear clinically

However we may ask for an X-Ray or MRI scan to confirm the diagnosis and rule out other associated injuries.

  • What are the treatment options for PCL tear?

Isolated PCL injury are treated with non surgical treatment like rest,Immobilization using knee brace,Physical therapy and activity modification.

If non surgical treatment fails to relieve your symptoms then surgery is eminent.

  • When do you need to undergo arthroscopic surgery?

    If you have persistent instability.

If you have combined injuries. Like knee dislocation and torn multiple ligaments surgery is almost always necessary.

Pain due to isolated PCL tear symptoms not improving with non-operative treatment like rest, immobilization with brace and physical therapy then we recommend Arthroscopic PCL reconstruction..

  • Surgical procedure

We are going to replace your tornedPCL with the new tendon graft taken from your own leg on the same side of the injured knee or opposite side.

This procedure is done under anesthesia always by making small incisions on your knee joint and using tiny instruments and camera guidance.

Risks and complications

As with any type of surgery, there are risks with ACL surgery. In general, surgery may cause:

  • Bleeding
  • Infection
  • Blood clots (DVT)
  • Knee pain
  • Stiffness in your knee
  • A graft not healing well
  • A graft failing after you return to physical activity
Is the arthroscopic PCL reconstruction a day care surgery?

Yes it can be done as a day care procedure and you can go home on the same day. But we recommend to stay for at least 24hours in the hospital after surgery for your safety and pain management.

Care at Home

  • Before Leaving Hospital

  • 2 weeks to 6 weeks
  • 6 weeks to 6 months

Before Leaving Hospital

Talk with your surgeon about any precautions you may have after surgery. You will be discharged on the same day of surgery. You would be given a knee brace which avoids your knee bending.Your doctor advises to use ice packs to decrease the swelling and pain.

Upto 2 weeks after surgery

Your surgeon would recommend to change the dressing after 2 days of surgery. You will continue using knee brace.Depending on the type of surgery, your surgeon advises either to walk with full weight or not to bear the weight.Keep your leg elevated above the heart level with a pillow below your legs always to minimize the swelling.Ankle back and forth movements are also essential to decrease the swelling and pain.

You will be advised Home physical therapy and you should be able to resume to your daily activities by this time.

2 weeks to 6 weeks

Continue Home physical therapy and try to gain your knee movements completely.Your surgeon would allow you to walk with full weight by the end of 6 weeks without the knee brace.Schedule an appointment with your surgeon at 6 weeks.

6 weeks to 6 months

Continue physical therapy, you might have occasional swelling and pain that is common. Schedule an appointment with your surgeon he would examine your knee and advises on further care. You should be able to perform sports activities at the end of 6 months.

Frequently Asked Questions?

  • What is arthroscopic PCL reconstruction?

Arthroscopic PCL reconstruction is a surgical procedure to repair a torn posterior cruciate ligament (PCL) using an arthroscopic and synthetic ligament material.

  • Why is it done?

The procedure is done to restore stability and function to the knee joint after a PCL injury, and to alleviate pain and improve knee range of motion.

  • How is the surgery performed?

The surgery is performed through small incisions in the knee using an arthroscope. The torn ligament is removed, and the synthetic ligament is attached to the bones of the knee using screws or other fixation devices.

  • What is the recovery time?

The recovery time varies depending on the individual, but it typically takes several months to fully recover from the surgery. Physical therapy is often recommended to help regain strength and range of motion in the knee.

  • Are there any risks or complications associated with the procedure?

Like any surgical procedure, there is a risk of complications, including infection, bleeding, nerve or blood vessel damage, and implant failure. It's important to discuss the potential risks and benefits of the procedure with your surgeon before making a decision.

  • How successful is the procedure?

The success of the procedure depends on several factors, including the severity of the injury, the patient's age and overall health, and their commitment to rehabilitation. In general, arthroscopic PCL reconstruction has a high success rate and can provide good outcomes for patients.
